Once you inquire about Covenant Home Care’s home health care services, we immediately start working with you to secure available funding from Medicare or other available sources. Our services start with a home assessment by a registered nurse, under the directive of your physician, to fully understand your total care needs.
At
Covenant Home Care, we provide our patients with a comprehensive written
plan of care, fully outlining the costs and services we will provide.
We have a HIPAA-compliant established privacy policy in place to protect
your personal information.
Every month, a supervisory nurse will review your plan of care and evaluate if new needs have arose in the past month. If for any reason your care plan needs to be changed, it will be changed immediately.
We maintain ongoing communication with our clients’ physicians and clinics to provide an ongoing dialogue to facilitate and mange your health care needs.
